KVTSC Forms
The following information summarizes the forms that are typically required for each type of title transaction KVTSC processes. Instructions for completing each of the forms is also generally included within the form itself. In addition, it is mandatory that you utilize the must current Division of Vehicles form, complete all the information, and sign, and date the form(s) in order for KVTSC to process your transactions. For your reference, the most current version of state-approved forms can be found HERE.
Before processing any title work through KVTSC, you will initially need to complete the APPOINTING AUTHORIZATION FOR A DEALERSHIP OR TITLE SERVICE FORM. This form authorizes a dealership’s or title service’s Runner to act on behalf of a dealership/title service. This form only needs to be completed once and will suffice for all future authorized transactions.

DEALER TITLE ONLY
- Assigned title; or Court order; Or Bill of Sale for antiques vehicle or trailer under 2K Gross Vehicle Weight
- If title was not issued by Kansas, an MVE-1 or MVE-3 inspection form is required, as well.
- Title and Registration Application (TR-212a)
- Lien Release (TR-150), or lien release on lienholder’s letterhead with the same information that is on the TR-150, if applicable
- Power of Attorney (TR-41) or Kansas Secure Power of Attorney, if applicable (seeherefor help determining when a Power of Attorney is needed.)
- Application for Secured/Duplicate/Reissue Title (TR-720B)
- Lien Release (TR-150), or lien release on lienholder’s letterhead with the same information that is on the TR-150 , if applicable.
- Power of Attorney (TR-41) or Title Authorization Form (TR-134), whichever is applicable. (seeherefor help determining when a Power of Attorney is needed.)
- Lien Release (TR-150), or lien release on lienholder’s letterhead with the same information that is on the TR-150
- Title and Registration Application (TR-212a)
- Repossession Affidavit (TR-84) Follow the instructions on the Repossession Affidavit, TR-84 concerning additional documentation requirements
- Request for Access to Vehicle Records (TRDL-302)
SALVAGE TITLE OR NONREPAIRABLE/JUNKING CERTIFICATE
- Assigned title
- If title was not issued by Kansas, an MVE-1 inspection form, or if vehicle is a total loss a copy of the Kansas accident report is required.
- Title and Registration Application (TR-212a)
- Salvage Non-Highway, Non-Repairable Certification (TR-13)
- Lien Release (TR-150), or lien release on lienholder’s letterhead with the same information that is on the TR-150 , if applicable
- Power of Attorney (TR-41) or Kansas Secure Power of Attorney, if applicable (seeherefor help determining when a Power of Attorney is needed.)
POSSESSORY LIEN TITLE
- Title and Registration Application (TR-212a)
- Possessory Lien Affidavit (TR-85) Follow the instructions on the Possessory Lien Affidavit, TR-85 concerning additional documentation requirements.
- Odometer Disclosure Statement (TR-59), if the vehicle is 10 model years old or less
MECHANIC LIEN TITLE
- Title and Registration Application (TR-212a)
- Mechanic’s Lien Sale Affidavit (TR-96M) Follow the instructions on the Mechanic’s Lien Affidavit, TR-96M concerning additional documentation requirements.
- Odometer Disclosure Statement (TR-59), if the vehicle is ten (10) model years old or less
- Request for Access to Vehicle Records (TRDL-302)
- MVE-1 inspection form
TITLE CORRECTION (FEE VARIES – Please inquire with KVTSC for details)
- Affidavit of Correction (TR-292)
